Model Release Form

In consideration for 2 dozen free Suncakes in the flavors and quantities identified on Order Page, I irrevocably grant to The Healthy Baking Company, LLC., Heart Thrive and California Suncakes (collectively, "California Suncakes") the right to distribute, transmit, publish, copy, or otherwise exploit, in whole or in part, the photographs attached to this Photo & Sound Bites Submission, Order, and Release Form ("Photographs") on California Suncakes' World Wide Web site (suncakes.com) or in any other electronic or print medium. I represent, warrant, acknowledge, and agree that I am the person depicted in the Photographs, that I own the copyright to the Photographs, and that the Company may use and exploit the Photographs without identifying me as the subject.

In further and additional consideration, I release and discharge California Suncakes, it's parent corporation, affiliates, related companies, agents, and assigns from any claim or cause of action, for, among other things, copyright infringement, invasion of privacy, right of publicity, and defamation, whether now known or later discovered, arising from or related to California Suncakes' use and exploitation of the Photographs.

I represent and warrant that (check one)_____I am over the age of 18 years, or_____I am under the age of 18 years and the person signing this form on my behalf is my parent or lawful guardian.
